Advanced Care Planning
Do you have your Advanced Care Planning documents completed? One of the most important things you can do to advocate for yourself and your health is to complete this paperwork. You do not need an attorney; you only need two non-family members to witness.
An Out of Hospital DNR is a one page document that allows a person to decide that they do not want to be resuscitated.
A Medical Power of Attorneyis a legal instrument whereby a person chooses someone to make healthcare decisions for them when or if they are unable to make them for themselves. This document only becomes active once a person is incapacitated.
An Advance Directive is a document that states a person’s wishes regarding medical treatment in specific situations.
